Eric Malone, PT

Clinic Director

Eric Malone PT is a graduate of the University of Washington which he attended for both undergraduate and Physical Therapy school. Eric is proud to have founded Proliance Sports Therapy- Bellevue along with Rick Rieger PT in 2001. He has been recognized as a board certified Orthopedic Clinical Specialist by the American Physical Therapy Association. While Eric has worked with current and former athletes from every professional league and collegiate sport, his passion remains with treating high school athletes. Eric has volunteered at many eastside high schools including Bellevue, Issaquah, Bellevue Christian, Juanita, and Sammamish providing on field medical coverage for well over 20 years. As the practice continues to grow, his responsibilities have become more administrative leaving limited time for clinical care. Eric’s first love is spending time with his family but he also enjoys travel, golf, bird hunting, fishing and Husky Football.

Angie Crivello, PT, DPT

Angie graduated from the University of Washington with a Bachelor degree in Sociology and Society and Justice and where she was also a member of the Women’s Soccer team. She returned to UW for graduate school where she earned a Doctorate of Physical Therapy in 2006. Angie has been with Proliance since graduating and enjoys treating a variety of people including those recovering from surgery and those with sports or work-related injuries. Angie has a special interest in ACL injury prevention and has worked with a number of youth sports teams in implementing exercises and programs to reduce the risk of ACL injuries. Angie is an avid sports fan and enjoys spending time with family and friends, playing a variety of sports and traveling.

Brett DeYager, PT, DPT

Brett graduated from Azusa Pacific University in 2006 with a Bachelor of Science degree in Applied Health and then moved on to earn his Doctorate degree in Physical Therapy from the University of Puget Sound in 2013. Brett has since been certified in the Selective Functional Movement Assessment (SFMAc) and has experience working with high level athletes from the WNBA, college athletes, as well as high school and middle school athletes. Brett enjoys working with a variety of orthopedic injuries and impairments, including athletic injuries, pre/post-operative rehabilitation, as well as work-related injuries and Benign Paroxysmal Positional Vertigo (BPPV). Outside of work, he enjoys spending time with family and friends, being involved at his church, as well as sports, remodeling projects, and he currently sits on the board of directors for a nonprofit called Summit Initiative. 

Lori Mock, PT, ATC

Lori Mock is a sports and orthopedic physical therapist/athletic trainer with a mission to provide remarkable care to her clients.  Lori understands the unique risks and injuries of young athletes to professionals, including world class Olympians. In addition to caring for active individuals of all ages, Lori Mock is a medical provider for USA Ski/Snowboard Team and USA Diving Team.  Lori is a former collegiate swimmer, track and cross-country athlete which provides Lori with an in-depth understanding of injury prevention, training needs and treatment of these athletes.  Lori is a co-founder of Seattle Pediatric Sports Medicine which is an organization of medical professionals focusing on education, research, policy formation, and collaboration in the care of pediatric and adolescent aged athletes.  In her free time, Lori enjoys skiing, swimming, running, cycling and outdoor activities.

Nicole Jimenez, PT, DPT, LAT, ATC

Nicole is a physical therapist and certified athletic trainer. She graduated from the University of Michigan with a Bachelor of Science degree in Kinesiology and a major in athletic training (Go Blue!). She earned her Doctorate in Physical Therapy from the University of Washington and has worked for Proliance Sports Therapy and Rehabilitation since. Nicole enjoys treating a variety of injuries and movement related dysfunctions including sports injuries and post-operative rehabilitation. She has experience working with youth, high school, and collegiate athletes and is passionate about injury prevention to maximize athletic participation. Nicole is originally from Puerto Rico and is fluent in both English and Spanish. In her free time, Nicole enjoys running, hiking, traveling, and spending time with friends and family.

Nelson Thai, PT, DPT

Nelson graduated with a Bachelor of Science in Biology from Eastern Washington University, and received his Doctorate in Physical Therapy from the University of New England. After extensive training and harsh winters in New England, Nelson comes equipped to treat a variety of conditions. During his time there, he was involved with providing graduate level mentorship to 1st year DPT students and gained valuable experience in orthopedic physical therapy and sports medicine rehabilitation. Nelson is passionate about empowering his patients to understand pain and their body’s resilience and amazing capability to heal. He enjoys helping people get back to their sport or activities they live for, as an avid skier himself, he understands the importance of being able to do the physical activities that you love. Nelson is committed to providing the highest level of care for all his patients. In his spare time, he loves to explore all kinds of food, rock climb, mountain bike, hike, and ski the PNW powder.

Stephanie Deye, PT, DPT

Stephanie graduated with a Bachelor of Arts in Psychology and a minor in Spanish for the Health Professions at Marquette University. She received her Doctorate in Physical Therapy from Marquette University in 2017. Stephanie spent the first five years of her career working in outpatient physical therapy in Glendale, Arizona. She specializes in treating various orthopedic needs and sports-related injuries. She also has experience with neurological, amputee, and vestibular rehabilitation. She is passionate about creating individualized programs utilizing exercise, patient education, and manual therapy to assist in overcoming movement dysfunction. In her spare time, she enjoys spending time outdoors, training for marathons, traveling with family and friends, and watching professional football. She values movement and finds joy in enabling her clients to meet their personal and functional goals.

Lisa Devlieg, PT, DPT

Lisa earned a Masters of Physical Therapy from the University of Washington in 2001. She has worked in the outpatient orthopedic setting on the Eastside for the past 20 years. She has worked with numerous spine, shoulder, knee, ankle and hip pathologies and she excels in post-op rehab. However, she has a special interest of treating adolescent idiopathic scoliosis using her training in the Schroth method. While her professional practice has included a wide array of experiences, she is most delighted by the opportunity to instruct, empower and inspire her patients along their rehab journey. Education is a vital component and Lisa clearly imparts a working knowledge of her patient’s specific dysfunction and the plan to address that. When she isn’t at work, you will most likely find her on local hiking trails. She is an avid year-round hiker and is blessed to live in such a beautiful area. She is also active in her local church, outreach projects, and has traveled to Bulgaria several times fighting against human trafficking.

Jacqlynn Zier, PT, DPT

Jacqlynn earned her Bachelor of Arts in Molecular Biology with honors at Colgate University and her Doctor of Physical Therapy degree from Eastern Washington University with the highest class GPA. She is passionate about helping people enjoy the activities they love, whether returning to competition following a sports injury or enjoying a daily walk after a joint replacement. Jacqlynn is excited to work with athletes of all ages and capabilities, and with a background in rowing, soccer, and trail running, she is particularly equipped to help athletes in those disciplines. She also has experience with balance training and fall prevention. Jacqlynn grew up on Orcas Island and currently lives in Kirkland with her husband and their rambunctious cat, Freddy. She spends most weekends with her family on Orcas and enjoys running, cooking, and reading about science and history.

Mark Bromberg, MSPT

Mark graduated from Minnesota State University-Mankato with a Bachelor’s degree in Athletic Training. While in the program he traveled with the men’s football and basketball teams and the women’s track team. He attended graduate school at Washington University in St Louis where he earned his Master’s degree in Physical Therapy. Mark has treated orthopedic injuries for the entirety of his career and worked with athletes from the professional ranks to weekend warriors with a special interest in shoulders and throwing athletes. Mark is an avid sports fan, maintaining his allegiance to Minnesota where he was born and raised. In his free time he enjoys playing board games with his family, trail running, golfing, playing drums and attending concerts.

Jody Bal, PTA

Jody has been with Proliance Sports Therapy of Bellevue nearly since its inception, joining the team in 2002. She graduated from Western Washington University with an Exercise Science degree in 1998. While first working as a Physical Therapy Aide for many years, she became licensed as a Physical Therapy Assistant earning her degree certification through Whatcom Community College in 2010. As one of the longest tenured providers at Proliance Sports Therapy, she has vast experience with all types of orthopedic patients but particularly enjoys working with post-surgical and sports related injuries. In her free time, she enjoys spending time with family and friends. She has a love of travel, playing tennis and watching sports, especially Seahawks football.

Rachel Caalim, PTA

Rachel attended Washington State University where she received a Bachelor of Science in Psychology with a minor in Human Development. After graduation, she enrolled in the Physical Therapist Assistant program at Pima Medical Institute and graduated with honors in 2011. Rachel primarily has worked in outpatient orthopedic settings treating those with post-surgical and sports related injuries. She enjoys working with patients of all ages, but has special interest in treating patients who have undergone joint replacement. When she is not working, Rachel can be found spending her free time with family and friends, hiking, traveling, and playing softball.

Leo Santana, PTA

Leonardo (Leo) is a Seattle native who has been practicing as a Physical Therapist Assistant since 2018 and graduated from Pima Medical Institute in 2017. Leo has experience working in an outpatient setting treating post-surgical and sports related therapy. Leo is also a licensed massage therapist and has been practicing since 2012. Leo is Mexican/American and is fluent in both English and Spanish. When Leo isn’t working, he enjoys spending time with friends, family, traveling or working his weekend job as a DJ.

Maria Caballero, PTA

Maria Caballero earned her Bachelor of Arts degree in Psychology at Gonzaga University, as well as her AAS degree from the PTA program at Lake Washington Institute of Technology. She believes in a patient-centered approach to rehabilitation, utilizing exercise in order to achieve individual goals, return patients to their optimal function, and improve quality of life. Maria’s clinical interests include sports medicine, orthopedics, manual therapy, and pelvic health. She is passionate about working with athletes of all ages wanting to return to sport, treating the prenatal/postnatal population, as well as patients recovering from orthopedic surgery. When she isn’t at work, Maria can be found cooking, spending time with her friends and family, or traveling. As a Pacific Northwest native, Maria loves hiking, exploring the local restaurant scene, and is an avid Seattle sports fan.

Neill Smith, PTA

Neill attended Central Washington University where he received a Bachelor of Science in Clinical Physiology. After graduation he enrolled in the Physical Therapist Assistant program at Pima Medical Institute where he graduated in 2022 and started practicing shortly after. Neill has primarily worked in the Outpatient setting treating those with post surgical and sports related injuries. When Neill is not at work, you can find him taking his dog for walks down to Alki Beach, playing dodgeball or volleyball at the gym, or hanging out at a Mariners game.

Laura Browne has provided excellent and effective physical therapy to both myself and my husband in the past. Now, I am thankful to have her professional knowledge and caring treatment as I struggle with the dual problems of surgery recovery and recurrence of significant lymphedema near the surgical site. Laura fluidly adapts to the limitations that the lymphedema imposes on a “normal” path to surgery recovery, always finding effective exercises to substitute for ones that are not practical for the situation. Throughout, her communication is informative, honest, and encouraging. Laura Browne is a credit to the high standards Proliance seeks in their employees.

Kudos to Proliance Sports Therapy & Rehab for the superb treatment I’ve received over the past several months. A spring ski mishap resulted in my fractured arm near my shoulder and that led to surgery requiring a steel plate with several pins; in the same fall, I tore my rotator cuff in my ‘other’ arm. With both arms basically ‘out of commission’ I was scared and worried about how I’d ever use my arms normally again. Laura has amazed me with all of her knowledge and capability as she has led me through a PT Program that now has me functioning as normally as I was before… I am so thankful that I chose Proliance Sports Therapy for my rehab; should I ever need rehab again, I would choose Proliance in a heartbeat!

14 months ago I had total shoulder replacement on my right shoulder at Issaquah Proliance Orthopedics & Sports Medicine. Surgery went great! Started Physical Therapy 1 week later. My Therapist (Cathy Van Lith) discussed my goals with me and set a plan. Everybody is so friendly and nice and make you feel welcome – you feel like family. I still have a long way to go but I know I couldn’t be in better hands. I have friends that have recommended that have gone and they can’t thank me enough for the recommendation.

Only place I’ll go: In the past, we’ve had a hard time finding good medical care. My wife found Proliance via recommendation from her OB who had his hip replaced at Proliance. We have since had 2 surgeries each that have required intense physical therapy at Proliance in Issaquah. The net result is that I won’t go anywhere else in the future. My expectations have been exceeded by both the care I’ve received and the support to get through the PT. Sounds silly I suppose, but I feel like I’m treated like a person, not just a body part that needs fixing. I’ve worked with Emily specifically and her PTs are truly a standout, they are awesome, pushing you while supporting, plus have been completely connected with my surgeon and with each other. If you need care, this is a good choice.

Patrick understood how difficult it was for me to be sidelined and how frustrated I felt. He pushed me to work harder and celebrated with me when I would hit a goal. Patrick was realistic about timelines, knew when to push me and when to hold me back so I didn’t injure myself again. He also did a great job of making our sessions fun and different. Kyle Mark was amazing! He patiently rehabbed my knee back and specifically researched exercises that would support my love of tennis, after I tore my meniscus. I left after the expected period of time and was able to resume tennis. Unfortunately, I caused an additional issue within the knee while hiking shortly after recovery and, even though I had moved out-of-state, he suggested exercises to get me back on track. I am 100% back to normal and so thankful that he took the time and shared his knowledge with me. Proliance is lucky to have such a smart, caring and dedicated physical therapist in their office.

 

After finger surgery four years ago on my left hand and two years ago on my right hand. Stephanie helped me through many therapy appointments. She also instructed me on home exercises. She is very professional and will go the extra mile to get the results we both wanted. I would highly recommend her.
