Information for New Patients

Thank you for choosing Proliance Sports Therapy and Rehabilitation of Bellevue and Issaquah. 

To schedule an appointment please call our friendly reception staff. It is helpful if you have the following information available:

  • Type of injury
  • The name of the referring physician
  • Your insurance card

If you require specific days or times, or would like to request a specific therapist, please notify our front desk staff at this time.

For your first appointment:

  • You may download our New Patient Forms - Bellevue
  • New Patient Forms - Issaquah and fill them out prior to your appointment. Plan to arrive ten minutes early to complete your paperwork.
  • Please bring the referral from your physician, your current insurance card, a photo identification (for parents/guardians of minors), and a form of payment for any co-pays.
  • You should plan to spend approximately one hour at each of your appointments.
  • Please bring exercise clothing and athletic shoes to your appointment. Loose fitting clothes are helpful for the initial evaluation.

General Information and Clinic Policies:


  • We understand that you lead a busy life. Therefore, we will do our best when scheduling appointments to accommodate you. We recommend that you schedule additional follow up visits in advance in order to guarantee that you can get the appointment times that fit your schedule.
  • We believe that communication with your Physical Therapist is of utmost importance to the quality of care that we deliver; therefore it is our policy to schedule all your appointments with the same therapist. 

Cancellation and Reschedule Policy

We ask that you notify us with at least 24 hours notice if you are unable to keep your appointment.


Your time in Physical Therapy is important to your recovery. Therefore, we ask that you do not bring in your children with you to your appointments as this distracts from the quality of care that you and our other patients receive.


While physical therapists enjoy direct access in the state of Washington, meaning you can see a physical therapist without a referral from your physician, your insurance company may require that you have a referral in order for us to bill them for our services. Some insurance companies also require pre-authorization that may take up to 48 hours to complete.


Please provide us with your current insurance information when you call to schedule your first appointment. Benefit information is specific to each provider, plan, and member. We will call to verify your coverage and benefits before you are seen. This information is provided as a courtesy and does not guarantee payment. We strongly recommend that you contact your insurance company to verify your outpatient physical therapy benefits. For additional information please contact our front desk staff or our billing office.

We will also bill your open and allowed claim for:

  • Department of Labor andIndustries
  • Sedgwick CMS
  • Broadspire
  • Other Self-Insured Plans
  • Personal Injury Protection (PIP) – Automobile claims
